Steps for placement of Holter Monitor in ED:
  1. After speaking with Sanger ACP, ensure they have provided you with the name of the Sanger cardiologist who is on call that day which is who the patient will follow up with.
  2. Fill out and Fax the following form to the EKG tech (ASCOM #52380):
